- Find a place to practice
- Get liability insurance—you must have your own professional liability insurance of at least $1 million per occurrence/$3 million aggregate to become a VSP provider
- Obtain your NPI number
- Begin the medicare enrollment process (see our 41 Step Guide to Become a Medicare Provider w/ Screenshots)
- Register with CAQH
Step 2: Get in touch with Vision Plan
- Visit Vision Plan of America’s website
- Email: info@visionplanofamerica.com requesting optometrist enrollment information
- They will email you a packet of information describing plan details and steps required for enrollment
- Fill in and return:
- Agreement form
- Practice questionnaire
- Copy of current state license
- Current certificate of malpractice insurance
- W9 tax form for each provider in the practice
- You will receive a reply once credentialing is complete
