EyeQuest is a part of DentaQuest, the largest dental benefits administrator in the country, primarily for Medicaid. EyeQuest was launched in 2008, and now provides coverage for over 1.5 million individuals in the US.

Step 1: Complete the introductory requirements

In order to become an EyeQuest provider, you must:

Find a place to practice
Get liability insurance
Obtain your NPI number

Register with CAQH
